When it comes to your health information, you have certain rights. This section explains your rights and some of our responsibilities to help you.
Our fees are the same for all patients, insured or not. We are happy to file insurance claims on your behalf, including MetLife, Delta, Aetna, Cigna, UHC, and Superior, but patients are responsible for understanding their coverage. Any unpaid balance after insurance payment is the patient’s responsibility. Treatment estimates are provided to help you understand expected insurance coverage and your portion.
We require at least 24 hours’ notice for appointment cancellations. Missed appointments without proper notice will incur a $25 fee. Repeated cancellations may require full pre-payment before scheduling future visits.
To provide the highest level of professional care while maintaining fair fees, we offer the following payment options for our patients:
Estimated payment is due at the time of service. We accept Visa, Mastercard, Discover, cash, and personal checks. Any remaining balance after insurance payment, due to differences in coverage or usual and customary rates, is the patient’s responsibility. We use secure automated billing and payment platforms to ensure safe and efficient processing.
We also offer CareCredit, a dental financing plan that allows qualified patients to pay over time. Applications can be completed in our office or online. Our staff is happy to answer any questions regarding payment options.
Please note, we do not accept Molina, CareSource, or Medicaid.
